45 Y/O WITH SLURRING OF SPEECH

  This is an online E-log entry blog to discuss and understand the clinical data analysis of a patient, to develop competency in comprehending clinical problems, and providing evidence- based inputs in order to come up with a diagnosis and effective treatment plan to the best of my ability. A 45 year old female, R/O Doomipaamulu, came with the chief complaints of  1. Slurring of speech since 1 day 2. Double vision since 1 day 3. Swelling and numbness of tongue since 1day The patient was apparently asymptomatic 10 years ago when she had 2 episodes wherein she would not respond to anyone around her, which lasted about 5 minutes each. On going to the hospital, she was diagnosed with HTN for which she has been taking amlodipine. She was also diagnosed with DM 6 years ago. Following a hysterectomy, her sutures were not healing and on going to the hospital, she was found to be diabetic. 90 Y/O WITH SWELLING OF TONGUE

  This is an online E-log entry blog to discuss and understand the clinical data analysis of a patient, to develop competency in comprehending clinical problems, and providing evidence- based inputs in order to come up with a diagnosis and effective treatment plan to the best of my ability. A 90 year old female, R/O yerramguda came with the chief complaints of: 1. Slurring of speech since 1 day 2. Swelling of tongue since 1day 3. Fever since 1day The patient was apparently asymtomatic 3 months back when she developed fever, which was high grade, associated with chills and rigors, and decreased on taking medication. The fever subsided after 4 days. Along with the fever, she also developed redness in the mouth, with no other symptoms as mentioned by her.  2 months ago, she slipped on the bathroom and fell, which caused a fracture to get right femur. 65 Y/O MALE WITH SOB, CHEST PAIN, FEVER

A 63 year old male, R/O Nalgonda, farmer by occupation came with the chief complaints of: 1. Shortness of breath since 10 days 2. Chest pain since 10 days 3. Fever since 2 days Patient was apparently asymtomatic 25 years back when he started getting joint pains. They started with his right wrist first, followed by right elbow, then left wrist, then left elbow, then cervical joint, then B/L DIP, then B/L PIP, then B/L knee joint, then B/L ankle joint.  The pain increased in the morning, and decreased after some activity, around half an hour. After consulting a local RMP, he was put on NSAIDs and steroids, which he started taking. 10 years ago, he fell from a bike but sustained no injuries.   2 years ag

44 Y/O MALE WITH SHORTNESS OF BREATH

A 44 year old male, farmer by occupation cane with the chief complaints of 1. Shortness of breath since 10 days Patient was apparently asymptomatic 10 years back when he was diagnosed with CKD and has been on regular medication ever since.  Additionally, he was diagnosed with DM 5 years back when he went to the doctor with C/O stabbing-type chest pain, for which he has been taking glimepiride.  He was also diagnosed with HTN 1 year back after he had an episode of dizziness with blurring of vision and tachycardia.
