40Y/F WITH GIDDINESS

A 40 year old female, resident of Bachupalli, whose occupation is to water plants in a nursery, came with the chief complaints of: 1. Giddiness since 1 day The patient was apparently asymptomatic 6 years ago when she noticed that she had lost some weight. Additionally, she had 2-3 episodes of nausea within a week and hence consulted a doctor. Upon investigation, she was found to have gallstones. The doctor advised against surgery at that time, and hence, she was given medication (what medication?). She continued to take these tablets for 2 years. 4 years ago, the patient suddenly developed right sided loin pain, which was radiating towards the back, persistent in nature, and not associated with fever, nausea, burning micturition, hematuria or pedal edema. The pain lasted for 10 days after which she consulted a doctor. Initially, she was given medication for a week for the same and asked to follow up after one week. A 65 year old male, labourer by occupation, r/o gudigade came with the chief complaint of: 1. Shortness of breath since 3 days Tha patient was apparently asymptomatic 3 years back when he developed pedal edema uptil the knee, for which he went to the hospital. On performing investigations, he was diagnosed to be hypertensive, and he was told that his kidneys were failing, and dialysis was suggested.  Following this, they went for a second opinion to Khammam, where the same diagnosis was made. Hence, they came to our hospital to start dialysis. They had been getting hemodialysis done since 3 years- two times a week. No previous complications had been noted. 18Y/F WITH FEVER, JOINT PAINS

Fever since 3months 2. Joint pain since 3 months 3. Headache since 3 months 4. Vomitting episodes since 10 days Patient was apparently asymptomatic 3 months ago when she developed fever, which was persistent in nature, low grade. However, one-two times a week, she would have exacerbations if the fever, wherein the fever would suddenly become high grade and associated with chills and rigors. It would decrease on taking medication e, high grade.  Once the fever subsided, the patient started having pain in the small joints, for which she took medication.
